Integration and construction of organizational culture: A case study on a restructured hospital in China

Para manter a sua competitividade e desenvolvimento as organizações necessitam de
inovação e reformas. A reorganização dos hospitais, enquanto forma de
mudança organizacional, traz consigo mudanças organizacionais internas e
externas que podem dificultar a sobrevivência e o desenvolvimento dos
hospitais. Um dos problemas mais importantes da gestão é a integração da
cultura organizacional.
Esta pesquisa tem como objetivo realizar um caso de estudo de um hospital Chinês
que tem vindo a ser reestruturado nos últimos dez anos e analisar a influência
da integração e construção da cultura organizacional na sobrevivência e
desenvolvimento dos hospitais Chineses reestruturados.
Esta tese divide-se em três partes; (a) o processo e os problemas principais da
reorganização hospitalar; (b) características da cultura organizacional na fase
inicial da reorganização hospitalar; (c) o processo da integração e reconstrução
da cultura organizacional na reestruturação hospitalar e as características da
nova cultura organizacional após dez anos de reorganização.
As principais conclusões desta tese são as seguintes:
1. Processo e principais problemas da reorganização hospitalar: a reorganização
do hospital F, realizada segundo instruções governamentais, cobre
essencialmente a questão da propriedade, a estrutura organizacional, os
recursos humanos, as estratégias de desenvolvimento e cultura organizacional.
Os principais problemas após a reorganização hospitalar são, a escassez de
fundos e de pessoal assim como de conhecimento. Contudo a reorganização
hospitalar pode ser considerada como boa e útil.
2. Características da cultura organizacional na fase inicial da reorganização
hospitalar: a cultura organizacional nesta fase é fluída pouco orientada. Como
resultado, as pessoas tendem a resistir e a esperar passivamente, levando à

degradação do serviço prestado pelo hospital. Com a bem sucedida
reorganização hospitalar, as pessoas irão gradualmente conformar-se ou
adaptar-se, apoiar e aderir à nova cultura organizacional.
3. Integração e reconstrução da cultura organizacional após a reorganização: (a) a
integração e reconstrução da cultura organizacional do hospital F cobrem três
aspetos: a cultura espiritual, a cultura comportamental e a cultura material. A
estrutura sénior de gestão, responsável pela concepção desempenha um papel
muito importante; (b) as características da nova cultura organizacional, após
dez anos de reorganização hospitalar incluem: relação interpessoal harmoniosa,
forte coesão organizacional, orientação de longo prazo e melhoramento
contínuo. A nova cultura organizacional contém muitos elementos da cultura
tradicional chinesa; (c) num período de oito a dez anos a reorganização do
hospital F passou por períodos de euforia, choque cultural, aculturação e
estado estável. O choque cultural atingiu o seu máximo entre dois e cinco anos
após o início da reorganização e durou entre dois e três anos; (d) a ocorrência e
o desenvolvimento da integração e reconstrução cultural dos hospitais
reestruturados estão fortemente relacionados com a sua cultura organizacional.
Os dois influenciam-se mutuamente.

Organizations need innovation and reform to maintain their competitive edge and
development. Hospital reorganization, as a form of organizational change, tends to
bring both internal and external fundamental changes, causing many problems and
contradictions that may directly hinder the survival and development of hospitals. One
of the inevitable deep-rooted management problems is organizational cultural
integration.
This research aims to make a case study of a hospital in China which has been
restructured for ten years, and to make a primary research on the influence and
significance that organizational cultural integration and construction have on the
survival and development of Chinese restructured hospitals.
This research is divided into three parts: (a) process and major problems of
hospital reorganization; (b) characteristics of organizational culture at the initial stage
of hospital reorganization; (c) process of cultural integration and reconstruction of
hospital restructuring, and features of new organizational culture after 10 years of
hospital reorganization.
Main conclusions of this research:
1. Process and major problems of hospital reorganizations: Reorganization of F
Hospital, conducted under government order, mainly covers management ownership,
finance, organizational structure, human resource, development strategy and
organizational culture. Major problems after hospital reorganization are lack of funds,
and personnel as well as management. Nevertheless, hospital reorganization is good,
positive and progressive.
2. Characteristics of organizational culture at the initial stage of hospital
reorganization: The organizational culture at this stage is loose and disoriented. As a
result, people tend to resist and wait passively, leading to inability of the hospital.With successful hospital reorganization, people will gradually conform or adapt to,
support and embrace the new organizational culture.
3. Integration and reconstruction of organizational culture after hospital
reorganization: (a) the organizational cultural integration and reconstruction of F
Hospital mainly covers three aspects: spiritual culture, behavioral culture and material
culture. The hospital’s senior management structure, of which the design concept
often originates from the hospital staff, plays a vital role; (b) characteristics of new
organizational culture after ten years of hospital reorganization mainly include:
harmonious interpersonal relationship, strong organizational cohesiveness, long-term
orientation, ceaseless self-improvement, and culture of learning. The new
organizational culture contains rich China’s traditional cultural elements; (c) the
reorganization of F Hospital has experienced periods of euphoria, cultural shock,
acculturation and stable state, lasting at least eight to ten years in total. Cultural shock
reached its summit after two to five years since hospital reorganization (lasting about
two to three years). The cultural integration of a restructured hospital in this case
mainly adopts integration mode and penetration mode; (d) the occurrence and
development of restructured hospitals are closely related to their organizational
culture. The two impact and reinforce each other.
