Fazer a história da forma como a sociedade e o poder lidaram com os que eram definidos como loucos é uma tarefa complexa. Tendo em conta que se trata de uma análise que obrigatoriamente relaciona várias áreas e a sua escrita tanto pode dizer respeito ao historiador como ao médico, ao filósofo, ao psicólogo ou ao psicanalista. Deste modo, existem várias e longas leituras sobre este tema vindas de diversos campos do saber. Sabe-se que foram as primeiras décadas do século XIX que marcaram o começo da moderna psiquiatria, quer pela convergência do reconhecimento médico da doença mental como uma afeção natural, suscetível de ser estudada e tratada, quer pelo surgimento de um movimento social que levou à criação de uma instituição especializada para a assistência aos doentes mentais. O primeiro hospital psiquiátrico português – o Hospital de Rilhafoles – abriu portas em 1848, quando o segundo hospital a ser aberto em Portugal, o Hospital Conde de Ferreira, no Porto, apenas o concretizou em 1883. Durante quase meio século, o Hospital de Rilhafoles foi o único estabelecimento para alienados em território português, enfrentando, como principal obstáculo, o problema da sobrepopulação. O principal objetivo desta dissertação é contribuir de forma mais precisa para a história do surgimento da psiquiatria em Portugal, com o maior detalhe cronológico possível. O foco principal incidiu sobre a análise das várias administrações que passaram pelo Hospital de Rilhafoles durante a segunda metade do século XIX, privilegiando a análise dos relatórios que os vários diretores deste hospital foram concretizando.

Revealing the history of the way that society and power handled with those classified as insane it's a complex task. Considering that it's about an analysis that obliges relating several areas of study and its writting may concern the historian as much as the medic, the philosopher, the psychologist or the psychoanalyst. Thus, there is plenty and lengthy literature on this subject coming from the most diverse fields of knowledge. It is known that the first decades of the XIX century marked the beginning of modern history of psychiatry, with the medical acknowledgment of mental illness as a natural affection, susceptible of being studied and treated, but also with the emergence of a social movement that led to the creation of a specialized institution to assist mentally ill patients. The first portuguese psychiatric hospital - Hospital de Rilhafoles - opened doors in 1848, whilst the next hospital to open in Portugal, the Hospital Conde de Ferreira, in Oporto, only made it in 1883. For nearly half a century, the Hospital de Rilhafoles was the only institution for alienated people in portuguese territory, while facing, as the main obstacle, overcrowding. This thesis’ goal is to contribute in a more precise way about the history of emergence of Psychiatry in Portugal, with as much chronological detail as possible. The main focus fell over the analysis of various administrations that managed Rilhafoles in the second half of the XIX century, with special focus on the analysis of the reports that the various directors of this hospital materialized.
